Exercise physiology services work in tandem with other professions to assist in the long-term management of chronic health conditions and musculoskeletal disorders by providing evidence-based exercise interventions to target many conditions. Through program development and lifestyle changes, our team of Accredited Exercise Physiologists will tailor interventions to suit your individual needs and goals.

Exercise Physiologists can

  • Educate clients on general health and well-being, conditions, injuries and how to best manage these conditions and injuries
  • Conduct assessments to establish current capacities, health, and fitness and identify any areas for improvement
  • Prescribe individualised exercise programs to treat conditions and improve health and performance while avoiding further complications
  • Identify other areas of concern and refer to other allied health professionals when necessary
  • Provide rebates for Exercise Physiology by private health funds
  • Provide Team Care Arrangement services with a referral from GP Treat clients claiming through NDIS and DVA.

Musculoskeletal injury management and prevention

Injuries tend to occur when our body does not have a robust response to the desired movement pattern.

After identifying key areas requiring rehabilitation, our team will help design an injury management and prevention program that not only ensures the best outcomes according to current best practices but a program that suits your unique lifestyle and personality.

We utilise the AxIT force analysis system to assess your current force capacity and identify any imbalances or potential strength discrepancies which guide our individualised program creation. Exercise Physiologists at 4D Health and Performance provide individualised assessment, exercise advice, prescription, behavioural change counselling and support for people with chronic and/or complex disease(s). We believe people of any age can improve their exercise habits and receive benefits.

Metabolic diseases such as obesity, hyperlipidaemia, metabolic syndrome, polycystic ovarian syndrome, type 2 diabetes and type 1 diabetes disrupt the ability of the body to perform critical functions involving the metabolism of nutrients.

Customised evidence-based exercise programs designed and delivered by our Exercise Physiology team can help with:

  • Risk factor reduction for various metabolic conditions
  • Management of a range or combination (Metabolic Syndrome) of existing medical conditions

Our exercise physiology team is here to help you navigate your recovery, as well as manage the day-to-day symptoms of your chronic conditions. A customised exercise program that includes an Alter-G Anti-Gravity treadmill can assist with the management of cardiovascular diseases such as hypertension, coronary heart disease, heart failure, cerebral apoplexy (Stroke), intermittent claudication, pulmonary diseases such as chronic obstructive pulmonary disease, asthma, cystic fibrosis, musculoskeletal disorders including osteoarthritis, osteoporosis, back pain, rheumatoid arthritis as well as negative side-effects related to cancer and its treatment(s).

Chronic and acute pain

Pain has been proven to be reduced through exercise treatment. Our Exercise Physiologists can help identify and differentiate between the subcategories of pain – neurological; musculoskeletal; acute; chronic; delayed onset of muscle soreness (DOMs).

Our team will help you gain a better understanding of the movements and conditions that are potentially contributing to and causing your pain. Our Exercise Physiologists will help educate you on how and why you are feeling pain, and how to ensure/reassure the body is not under present or future harm. Our Exercise Physiologists are qualified to empower individuals to independently manage flare-ups of pain, prevent further tissue damage, and address concerns related to results from medical imaging/scans.

We aim to increase your confidence and ability to participate in regular physical activity, whilst improving sleep, mental health, diet and knowledge/understanding of pain.

Different types of pain Exercise Physiologists treat include:

Acute pain
  • Pain lasting less than 3 months
  • Typically involving the body’s response to damaged tissue (muscles/tendons/ligaments)
    • Eg: Shoulder/hip/knee or ankle-related impingement, inflammation, bursitis, tendinopathy
    • Torn or sprained ankle/calf/hamstring/groin/rotator cuff

Chronic pain
  • Pain lasting longer than 3-6 months
  • Typically involving pain in the absence of tissue (muscles/tendons/ligaments) damage, outlasting the normal/expected healing time
    • Eg: Disc pathologies/low back pain
    • Fibromyalgia
    • Radiculopathy/sciatica
    • Osteoarthritis

Neurological

Engagement in physical activity and movement is highly recommended to manage and rehab neurological health conditions such as Parkinson’s Disease, Multiple Sclerosis and Cerebral Palsy to prevent further loss of function and capacity.

Neurological conditions present differently in everyone and may affect multiple physical and physiological functions. Our Exercise Physiologists provide individualised exercise programs aimed to recover muscle strength, coordination, and fitness to enable you to get back to doing what you want and need to do and help improve your quality of life.

Walking training has a positive effect on gait function and walking capacity in patients with Parkinson’s disease and Cerebral Palsy. The Alter-G Anti-Gravity treadmill enables supported walking by reducing the impact of gravitational forces and body weight. Patients can benefit from programs that include the use of the Alter-G Anti-Gravity Treadmill which is customised to meet their needs depending on the stage and presentation of their condition.

Mental health

Exercise physiologists play an important role to assist people with mental health conditions: depression, stress, and anxiety.

  • Exercise can improve mood in people with diagnosed depression or depressive thoughts associated with chronic diseases (such as chronic pain)
  • Exercise assists with managing fatigue and loss of fitness from inactivity.
  • Exercise alongside other modifiable factors in life, counteracts weight gain which is one of the key side effects of medication that can negatively impact metabolic health.

High-performance training and program design for clients

Our team design individualised aerobic and strength-based programs for clients who aspire to increase their performance in their chosen sport. Our Exercise Physiologists are highly trained in enhancing performance via a thorough understanding of muscle/joint/neurological/psychological function.

Aside from rehabilitating sport-specific injuries, we aim to prepare individuals for the physical demands required in their chosen pursuit or sport, reducing the likelihood of future injuries. This ‘prehabilitation’ involves targeting joints/muscles/tendons/ligaments that are typically vulnerable in specific sports, allowing us to educate and eliminate popular mechanisms of injury.

We work with clients to identify biomechanical insufficiencies in Functional Movement Assessments and use evidence-based objective tools such as (AxIT) force-plate testing to highlight asymmetries between limbs in sport-specific movements. These minor details allow people to gain that 1% they need to take their performance to the next level.

Exercise Physiology and Medicare

You may be eligible for up to five appointments per calendar year with an accredited Exercise Physiologist under Medicare Chronic Disease Management (CDM) Plan. There is no list of eligible conditions. To get started, simply speak with your GP to see if you’re eligible for a CDM plan and get a referral to see an Exercise Physiologist. Bring a copy of your referral with you to your initial consultation to get a rebate for your approved sessions.

In your initial session, you will work with your Exercise Physiologist to go through a selection of assessments to evaluate the physical function as well as discuss comprehensive history and symptoms. This will allow us to provide a tailored exercise solution. By following a collaborative approach to healthcare, we provide timely updates to your GP to keep them informed of your progress and results.

Common chronic injuries and conditions that we deliver specific, individualised exercise therapy include:

As Medicare-recognised allied health practitioners, our Exercise Physiologists can help you navigate the difficult process of understanding your injury or health condition following your service history. The rehabilitation program is designed to give you the necessary support to regain your health and independence after a service-induced injury or illness.

Exercise Physiologists can help by assessing your current physical and social health. They will help identify and establish goals and develop a plan to manage the condition through appropriate exercise treatments. By following a collaborative approach to healthcare, we provide timely updates to your GP to keep them informed of your progress and results.

The DVA sessions are focused to help veterans with various conditions, including but not limited to:

  • Lower back pain
  • Joint reconstruction (Total knee/shoulder/hip replacements) and muscular injuries
  • Cardiovascular disease

  • Type-2 diabetes
  • Hypertension
  • Mental health support (Depression, PTSD)

Exercise Physiology and NDIS

We work collaboratively with the participant to construct a plan with an aim to provide the necessary skills to reduce the impact of living with a disability, improve life quality and increase the ability to perform activities of daily living. After the initial consultation, we will liaise with you, or your support coordinator/plan manager on the recommended treatment plan.
 
We accept and help NDIS participants as part of their treatment, including:

Initial consultation - $145
(45-minute session)

Our initial consultation is structured around an in-depth medical history and physical examination to correctly diagnose your injury. It will include the initial treatment and appropriate rehabilitation plan.

Standard Consultation - $115
(30-minute session)

A standard follow-up consultation is for existing clients of 4DHP and will involve a re-assessment to determine your progress, continued treatment and an update to your rehabilitation plan.

All our Exercise Physiologists are ESSA Accredited and are registered for private health fund rebates through on-the-spot HICAPS. The amount that your health fund will pay depends on the level of coverage on your plan.

Individualised Programming

  • 4-WEEK Program $150 per program
    Following your assessment you will receive an individualised 4-week program. Your 4-week program will allow you to monitor and track your progress independently.

  • Online Program - $35 per week
    Ongoing weekly online programming is provided through the TrainHeroic application which gives you direct contact with our Strength & Conditioning department. Allowing our team to make adjustments to your program as required.

Private Health funds and exercise physiology

All our Exercise Physiologists are ESSA Accredited and are registered for private health fund rebates through on-the-spot HICAPS. The amount that your health fund will pay depends on the level of coverage on your plan.

Exercise Physiology service rebates are offered to their members by several private health fund providers. No referral is required to access Exercise Physiology services through private health. The amount of the rebate will depend upon the individual’s health fund and level of coverage. Please contact your health fund provider to discuss the available rebate on Exercise Physiology services.
